Medicare Fraud Prevention Information
Senior Medicare Patrol (SMP) Program - recruits and trains volunteers to educate seniors to protect themselves against health care fraud, consumer scams, detection of fraudulent activities and assists with reporting suspected health care fraud to appropriate entities.

The program provides nutritionally balanced meals to eligible residents 60 years of age and over and their spouses. Meals are served through community cafes or delivered to the home-bound older adults.

CT Veterans requiring home and based services in order to remain in the community and avoid institutionalization have the opportunity to self-direct their own care. Through this program veterans serve as an employer and have the freedom to hire and fire their own Personal Care Assistants and with the help of a fiscal management service, manage their individualized care plans and corresponding budgets.

The Congregate Housing Services Program (CHSP) provides supportive services for participants in eligible housing projects who meet specific requirements. The supportive services may be provided to frail, elderly persons at least 62 years of age or non-elderly persons with disabilities for the purpose of promoting independence, preventing unnecessary institutionalization and helping people remain in the community as long as possible.

The State Department on Aging coordinates the "Live Well", Chronic Disease Self-Management Program (CDSMP) and holds workshops at various locations throughout the state. Live Well is targeted to older adults with chronic health conditions and helps them learn practical ways to deal with issues such as pain and fatigue, nutrition, exercise and talking with your doctor.
