• “Live Well” Chronic Disease Self-Management Program (CDSMP)

  The State Department on Aging coordinates the "Live Well", Chronic Disease Self-Management Program (CDSMP) and holds workshops at various locations throughout the state. Live Well is targeted to older adults with chronic health conditions and helps them learn practical ways to deal with issues such as pain and fatigue, nutrition, exercise and talking with your doctor.

  Support to help family caregivers find services for their loved ones. Provides an array of services like information, referrals to support groups and short-term respite in the home to help relieve caregiver stress.

• The CHOICES Program

  Connecticut's program for Health insurance assistance, Outreach, Information and referral, Counseling, Eligibility Screening. This program provides information to persons age 60 and older as well as to people with disabilities, and is comprised of both staff, in-kind professionals from local service agencies and volunteers.

• Alzheimer's Respite Care Program

  This program offers relief to stressed caregivers by providing information, support, the development of an appropriate plan of care, and services for the individual with Alzheimer’s disease or related dementia. Administered by the Department on Aging.

• Connecticut Mosquito Management Program

  The Connecticut Mosquito Management Program, a collaborative effort of five state agencies, is responsible for monitoring and managing the state's mosquito population to reduce the potential public health threat of mosquito-borne diseases. It uses a combination of education, surveillance, source reduction, larval and adult mosquito control and personal protective measures.

  CT AHEAD helps high school students with disabilities learn about support programs at different colleges in the state. AHEAD also advocates for the needs of students with disabilities and promotes programs to help them succeed in college.
